Student Success Projects are designed to utilize critical thinking skills required in health care professions. Each week you will be required to analyze a health care scenario and apply critical thinking skills. This will require use of your own words summarizing the project requirements. Please review the “Generative AI Use Policy” below for further instruction.
Student Success Projects are graded on two components: 1/ Project requirements and 2/ Personal reflection. Both components are required for every Student Success Project.
    Project Requirements:
Chapter reading and module lessons introduced employee care and patient safety. Chapter 11 identified 7 common health care workplace hazards.
    Chemical
    Environmental
    Ergonomic
    Infectious
    Physical
    Psychosocial
    Workplace violence
Consider a recent or current work environment. Were there hazardous conditions present, like those presented in readings and lessons? Keep that thought in mind to use while you create your ideal medical work environment, whether at a facility or remote setting.
    For this project:
Using at least 300 words, create and describe your ideal medical work environment that you would be happy to work in, and
Then identify in your summary at least three of the 7 hazards identified, and
How you plan to prevent that hazard in your ideal setting.
    Submit your project summary.
    Personal Reflection:
With a separate submission, reflect on the setting you created. Summarize with at least 100 words what you learned from this assignment in preparation for your health care profession. Submit your reflection.
All project submissions must include your name, the date, the course number, and the professor’s name. Two separate submissions are required.
Repeating the instructions, including required header information, or indicating a citation does not count towards the final word count requirement of the project. For further guidance, reference “Files” for sample projects, attend the live lecture or view the recording, or communicate with your course instructor.

Scenarios The following scenarios represent real-life examples of quality improv

Scenarios The following scenarios represent real-life examples of quality improvement tools (FMEA, PDSA, and RCA). You will select one scenario related to each quality improvement tool (FMEA, PDSA, and RCA) to complete this assignment. FMEA Scenario A: The medication administration process in the pediatric office where you work has become quite cumbersome with the addition of two new pediatricians. Additionally, the office recently added vaccine administration for pediatric patients. However, some parents took their children to the health department for vaccine administration. Staff are looking for ways to reduce the risk of administering the wrong medication. Scenario B: During a recent hospital merger, the birthing services of two hospitals were combined and located at one hospital. While the risk of infant abduction is statistically low, the newly formed hospital wants to ensure that the combined organizations have the safest environment possible for caring for newborns and limit the risk of infant abduction. PDSA Scenario A: In the skilled nursing faculty where you work, the incidence of pressure areas has increased. When you work on the weekends, you find that the preventative measures (heel boots, offloading pressure mattresses) are available but not frequently used. Scenario B: Patient satisfaction scores have remained unimproved related to patients who report that staff “always” explained about medicines before administering the medication. The nursing department believes they have a new process that will improve this patient satisfaction metric. RCA Scenario A A patient determined to be a fall risk was receiving care without any prevention strategies in place. The patient fell, suffered a subdural hematoma, and went to surgery for a craniotomy. Scenario B A patient underwent significant abdominal surgery and later was found to have a retained sponge in the abdominal cavity when they were readmitted with pain and sepsis. Directions From the Scenario list, choose one scenario for each QI tool (FMEA, PDSA, RCA). Use your chosen scenarios to complete the Module Two Activity Template, linked in the What to Submit section. Complete a comparison of QI tools using the details of your chosen scenarios. For each tool, identify a change theory that you feel best applies. You must cite at least two current scholarly sources. To be considered current, scholarly sources must have been published within the last five years. Specifically, you must address the following rubric criteria: Part One: FMEA QI Tool: Describe the FMEA QI tool associated with your chosen scenario. Consider the following in the response: The unique characteristics of the QI tool Situations for application of the QI tool The steps that may be included as part of the QI tool Purpose: Explain the purpose of the FMEA QI tool as it relates to your chosen scenario. Include the following in the response: A rationalization for the appropriateness of the QI tool to your chosen scenario Key connections between the scenario and the QI tool Change Theory: Discuss a change theory that would apply to your chosen FMEA scenario. Include the following in the response: A brief description of your chosen change theory Fundamental connections between the change theory and the scenario Part Two: PDSA QI Tool: Describe the PDSA QI tool associated with your chosen scenario. Consider the following in the response: The unique characteristics of the QI tool Situations for application of the QI tool The steps that may be included as part of the QI tool Purpose: Explain the purpose of the PDSA QI tool as it relates to your chosen scenario. Include the following in the response: A rationalization for the appropriateness of the QI tool to your chosen scenario Key connections between the scenario and the QI tool Change Theory: Discuss a change theory that would apply to your chosen PDSA scenario. Include the following in the response: A brief description of your chosen change theory Fundamental connections between the change theory and the scenario Part Three: RCA QI Tool: Describe the RCA QI tool associated with your chosen scenario. Consider the following in the response: The unique characteristics of the QI tool Situations for application of the QI tool The steps that may be included as part of the QI tool Purpose: Explain the purpose of the RCA QI tool as it relates to your chosen scenario. Include the following in the response: A rationalization for the appropriateness of the QI tool to your chosen scenario Key connections between the scenario and the QI tool Change Theory: Discuss a change theory that would apply to your chosen RCA scenario. Include the following in the response: A brief description of your chosen change theory Fundamental connections between the change theory and the scenario What to Submit Submit your completed Module Two Activity Template. Your submission should be a 2- to 3-page Word document with a separate reference page that is not part of the page count. Follow APA Style standards for formatting and citing your sources.

Step 1: Review the response to Exercise 2 listed below.
    “According to the Agency

Step 1: Review the response to Exercise 2 listed below.
“According to the Agency for Healthcare Research (2022), safety, efficiency, patient-centered, timeliness, efficiency, and equity are the six domains to ensure the best quality is provided to patients. Healthcare organizations, including healthcare management, must strive to meet these objectives to create a positive experience for the patients and the overall organizational culture. Failure to provide quality care can lead to unwanted, negative consequences such as distrust of the healthcare providers and or the company.
An excellent quality experience I had was during the process of establishing my son with a new pediatrician. My child’s previous provider was moving to a new clinic. Therefore, I had to be assigned a new pediatrician that was covered by our insurance. When the new pediatrician’s office called me to schedule the new patient appointment, they realized my child’s birthday was in a few weeks. Once they became aware of that, they discussed with me the option of scheduling the new patient appointment and the three-year checkup, at the same time. This choice was more effective for me and the clinic as we could get two appointments done at once. The decision also allowed me to only take one trip into town and make it accessible for my husband to attend.
Additionally, the excellent quality continued during the appointment. When the nurse was reviewing our information, she realized my child’s vaccination records were never uploaded by the previous pediatrician. I was able to find a copy of the vaccinations and she uploaded each vaccine and printed me a copy to verify that they were uploaded successfully. Upon doing so, she noticed two vaccines from his two months were missing. However, I knew my child had his two-month vaccines in a different clinic out of state. When I informed the nurse, she trusted me and asked for the previous clinic’s information so she could retrieve my child’s record. When we met the new pediatrician, the great experience continued. They were thorough in reviewing my child’s record, listening to the concerns we had, and providing us with a referral we had asked for before but never received.
Consequently, the negative quality experience I had was realizing our previous pediatrician clinic never submitted my child’s vaccine record. Their lack of attention to detail, made me lose trust in their ability to provide thorough patient care. Specifically, because the record I provided, was missing two vaccines and I was completely unaware. Furthermore, during another visit, my child had been submitted as the wrong gender which resulted in the incorrect growth chart being calculated. Although I am no longer a part of their system, I would not recommend any family to the clinic because I could not feel confident in their quality of care.
Management had a significant influence on both the excellent quality and the negative quality received. Patrice Spath states that management is responsible for how the organizational activities are “designed, carried out, coordinated, and improved” (2022). A strong organizational management can ensure that patients have excellent quality experience. However, if management does not provide top leadership to their employees, the organizational structure becomes compromised and results in poor patient care.”
Step 2: Review three systems models presented in chapter 3:
    Three Core Process Model
    Interconnected Systems Model
The Baldrige Performance Excellence Program Framework
Step 3: Choose the model you can best relate to at this time.
Step 4: Look at both your excellent experience and your poor experience, paying particular attention to how you described the manager’s role or influence. Transfer what you wrote about the manager’s role or influence to your paper.
Step 5: Now, think about those experiences from the perspective of the systems model you chose in question 3. Describe any additional insights gained about this experience by viewing it in the context of the systems perspective represented by the model.
Write a 2-page APA formatted paper by answering the questions listed above.

Hematopoietic: J.D. is a 37 years old white woman who presents to her gynecologi

Hematopoietic: J.D. is a 37 years old white woman who presents to her gynecologist complaining of a 2-month history of intermenstrual bleeding, menorrhagia, increased urinary frequency, mild incontinence, extreme fatigue, and weakness. Her menstrual period occurs every 28 days and lately there have been 6 days of heavy flow and cramping. She denies abdominal distension, back-ache, and constipation. She has not had her usual energy levels since before her last pregnancy. Past Medical History (PMH): Upon reviewing her past medical history, the gynecologist notes that her patient is a G5P5with four pregnancies within four years, the last infant having been delivered vaginally four months ago. All five pregnancies were unremarkable and without delivery complications. All infants were born healthy. Patient history also reveals a 3-year history of osteoarthritis in the left knee, probably the result of sustaining significant trauma to her knee in an MVA when she was 9 years old. When asked what OTC medications she is currently taking for her pain and for how long she has been taking them, she reveals that she started taking ibuprofen, three tablets each day, about 2.5 years ago for her left knee. Due to a slowly progressive increase in pain and a loss of adequate relief with three tablets, she doubled the daily dose of ibuprofen. Upon the recommendation from her nurse practitioner and because long-term ibuprofen use can cause peptic ulcers, she began taking OTC omeprazole on a regular basis to prevent gastrointestinal bleeding. Patient history also reveals a 3-year history of HTN for which she is now being treated with a diuretic and a centrally acting antihypertensive drug. She has had no previous surgeries. Case Study Questions Name the contributing factors on J.D that might put her at risk to develop iron deficiency anemia. Within the case study, describe the reasons why J.D. might be presenting constipation and or dehydration. Why Vitamin B12 and folic acid are important on the erythropoiesis? What abnormalities their deficiency might cause on the red blood cells? The gynecologist is suspecting that J.D. might be experiencing iron deficiency anemia. In order to support the diagnosis, list and describe the clinical symptoms that J.D. might have positive for Iron deficiency anemia. If the patient is diagnosed with iron deficiency anemia, what do you expect to find as signs of this type of anemia? List and describe. Labs results came back for the patient. Hb 10.2 g/dL; Hct 30.8%; Ferritin 9 ng/dL; red blood cells are smaller and paler in color than normal. Research list and describe for appropriate recommendations and treatments for J.D. Cardiovascular Mr. W.G. is a 53-year-old white man who began to experience chest discomfort while playing tennis with a friend. At first, he attributed his discomfort to the heat and having had a large breakfast. Gradually, however, discomfort intensified to a crushing sensation in the sternal area and the pain seemed to spread upward into his neck and lower jaw. The nature of the pain did not seem to change with deep breathing. When Mr. G. complained of feeling nauseated and began rubbing his chest, his tennis partner was concerned that his friend was having a heart attack and called 911 on his cell phone. The patient was transported to the ED of the nearest hospital and arrived within 30 minutes of the onset of chest pain. In route to the hospital, the patient was placed on nasal cannula and an IV D5W was started. Mr. G. received aspirin (325 mg po) and 2 mg/IV morphine. He is allergic to meperidine (rash). His pain has eased slightly in the last 15 minutes but is still significant; was 9/10 in severity; now7/10. In the ED, chest pain was not relieved by 3 SL NTG tablets. He denies chills. Case Study Questions For patients at risk of developing coronary artery disease and patients diagnosed with acute myocardial infarct, describe the modifiable and non-modifiable risk factors. What would you expect to see on Mr. W.G. EKG and which findings described on the case are compatible with the acute coronary event? Having only the opportunity to choose one laboratory test to confirm the acute myocardial infarct, which would be the most specific laboratory test you would choose and why? How do you explain that Mr. W.G temperature has increased after his Myocardial Infarct, when that can be observed and for how long? Base your answer on the pathophysiology of the event. Explain to Mr. W.G. why he was experiencing pain during his Myocardial Infarct. Elaborate and support your answer. Submission Instructions: Include both case studies in your post. Your initial post should be at least 500 words, formatted and cited in current APA style with support from at least 2 academic sources. No AI
